I think this is all an elaborate ruse. They are smart to know that if they try using a letter to prove he is alive that everyone will start to think he is dead, but that is exactly what they want. Create a little chaos, and then in a few weeks he comes back as the unkillable North Korean leader to his people.

I will be really concerned about the panic this would cause worldwide, if he does succumb to it.

I couldn’t imagine the panic if a major world leader dies.

In my amateur opinion, I don’t think it would be permanent. Because the condition causing the difficulty breathing is temporary, I would imagine that the use of the machine would be temporary also. The only way I could see this being permanent is if the body gets used to the breathing being done manually by the machine. Even then I would think that they could slowly decrease the machine’s strength, to build back the lung’s strength.

Even if he hasn’t given it to Phillip yet it shows, that even with the best security in the world, if you are contact with people you can get it.
