His leg is at risk from a diabetic foot that can even lead to amputation. Tutu (1984) and Gorbachev (1990) both 90 this year (Tutu's birthday will be in October) Frederik Willem de Klerk (1993) has cancer and I'd say is quite likely to be included next year. Martti Ahtisaari (2008) hasn't been seen in public for a year and went pretty quiet after catching covid in March 2020. Looked bad in paparazzi pictures from June 2020. A chance for next year as he used to be very active before covid. Adolfo Pérez Esquivel (1980) still looks great despite the fact that he'll also be 90 later this year.
