I think this is all an elaborate ruse. They are smart to know that if they try using a letter to prove he is alive that everyone will start to think he is dead, but that is exactly what they want. Create a little chaos, and then in a few weeks he comes back as the unkillable North Korean leader to his people.

In the video he seems to be limping, I checked some earlier footage but couldn't tell if he had the limp. 

The limp could an obesity caused waddle. 

I think you are on the right track with the limp. He is favoring his left leg, keeping the knee stiff as he swings it through, causing him to hike up his pelvis on that side to allow his foot to clear the ground.  If I had to put money on it, I would say a moderate knee injury, or perhaps even a minor surgery, like for a meniscal tear or something like that. But I would need pretty good odds.
