Nasty one to deal with when the lift car shoots up at maximum speed. 30 floors in 15 seconds. Maybe a broken counterweight. He hit the ceiling of the lift car. Maybe he should have instinctively fallen to the floor of the lift, in a aircraft type brace position, expecting it would be bound to smash into the upper winding gear.
