When King Michael was diagnosed in March 2016 nobody thought he'd make the starting line for 2017 let alone 4 months in and counting. Cell division is (generally) slower at 90 than 50, so that alone would contribute to a longer survival time; but 2. Nonagenarians typically are less resilient and have more co-morbidities that shorten their expected survival time and might also mean that doctors are not able to pursue an agressive course of treatment (intensive chemotherapy, extensive surgery etc.).
