'Going home to die' is typically used in conjunction with an incurable/terminal disease, like cancer, although even then, you would probably prefer to die in a hospice instead (as long as your deathbed could be surrounded by your loved ones). He spent 5 days in intensive care and was taken home in an Ambulance crewed by medics. That does tend to suggest that his health is at best, a bit fucked. Bush Snr. was admitted to the ICU just because he was 92, not because of chest pains and breathing trouble. Age plays a factor in everything at 89.
